Medical Social Work

COMMONWEALTH HOME CARE’s Medical Social Worker’s are available for intervention when you are receiving a qualifying skill (Physical Therapy, Speech Therapy, or Skilled Nursing) under your physician’s order. Our Social Workers are experts at assisting you with a variety of community resources to help you remain in your home environment and have a better quality of life.

Medical Social Services include but are not limited to:

• Short term counseling
• Financial assistance
• Coordination of community resources
• Placement assistance
• Medication assistance programs
