Our Mission

The mission of COMMONWEALTH HOME CARE is to provide compassionate, high quality, evidence-based care to clients in their homes in order to assist them in achieving the highest  level of function, safety, and independence possible.

This mission is accomplished by three primary means:

1.  We are owned and operated by clinicians with over 40 years of experience in health care who have treated patients. 

2.  We offer innovative specialty programs aimed at helping patients learn to manage their conditions independently.

3.  We focus on evidence-based care to make sure the interventions we do for our patients are highly effective.
